Si nos ponemos a revisar la literatura en relaci\u00f3n a la reganancia de peso post cirug\u00eda bari\u00e1trica, nos encontramos con diversos estudios que indican que \u00a0la baja de peso que se logra es de un 29% al 87%, dependiendo de la t\u00e9cnica quir\u00fargica elegida, seg\u00fan la evaluaci\u00f3n del equipo multidisciplinario y las comorbilidades asociadas, como diabetes, reflujo gastroesof\u00e1gico, h\u00edgado graso, entre otros.<\/p>\n

La cirug\u00eda, permite que el alimento llegue de una forma r\u00e1pida al intestino, donde se produce una secreci\u00f3n de hormonas las cuales tienen un efecto importante en la reducci\u00f3n del apetito, tr\u00e1nsito intestinal, resistencia a la insulina y control del az\u00facar en la sangre (glicemia), por lo que beneficia la baja de peso y la mejora de las comorbilidades antes se\u00f1aladas.<\/p>\n

\u00a0La reducci\u00f3n de peso ocurrir\u00e1 hasta los 12 a 18 meses posteriores a ella, \u00a0debido a la baja ingesta de calor\u00edas que tiene el paciente, generada por la restricci\u00f3n de volumen g\u00e1strico (espacio) que se genera despu\u00e9s de la cirug\u00eda. Adem\u00e1s, se disminuye la grhelina (hormona anorex\u00edgena), haciendo que nuestro organismo no sienta la necesidad de comer, ya que esta hormona es la encargada de indicarle al cerebro que debemos alimentarnos a distintas horas del d\u00eda.<\/p>\n

\u00a0Nuestro organismo se adapta a esta baja ingesta de energ\u00eda y comienza a utilizar otros sustratos como fuente de energ\u00eda, como es la grasa que va a estimular la lip\u00f3lisis corporal (usa la grasa como fuente energ\u00e9tica).<\/p>\n

La baja de peso debe estar guiada por profesionales del \u00e1rea de la salud, especialistas en cirug\u00eda bari\u00e1trica, con la finalidad de poder apoyar y ayudar psico y nutricionalmente al paciente en cada una de las etapas.<\/p>\n

La alimentaci\u00f3n post cirug\u00eda bari\u00e1trica, es espec\u00edfica seg\u00fan el tramo en el que se encuentre el paciente centr\u00e1ndose en una alimentaci\u00f3n variada, equilibrada, suficiente y responsable.<\/p>\n

Existen diversos factores que van a\u00a0 influir en la recuperaci\u00f3n del peso corporal despu\u00e9s de una cirug\u00eda exitosa. Debido a que posterior a la baja de peso, el metabolismo basal (energ\u00eda que se gasta en reposo) disminuye, lo que favorece la reganancia de peso. El ejercicio f\u00edsico, debe ir de la mano con la alimentaci\u00f3n, por lo tanto el sedentarismo favorecer\u00e1 que nuestro cuerpo no gaste energ\u00eda y comencemos acumular peso nuevamente.<\/p>\n

\u00a0Como bien vimos, en el articulo anterior, la conducta alimentaria del paciente, tambi\u00e9n esta asociada a esta Reganancia de peso post cirug\u00eda, ya que el descontrol alimentario permitir\u00e1 regresar a la conducta alimentaria que se ten\u00eda antes de la cirug\u00eda, lo que llevar\u00e1 a inexistencias de horarios, ayunos prolongado y mala selecci\u00f3n de alimentos. Se debe de considerar que el paciente comienza a tolerar mayor volumen y alimentos, generando cambios adaptativos que permitir\u00e1n el aumento de las anastomosis y adaptaci\u00f3n intestinal, por lo tanto podr\u00e1 ingerir mayor volumen de alimentos, lo que nos llevar\u00e1 a una ingesta cal\u00f3rica elevada, que va a generar acumulaci\u00f3n de peso, si es que este exceso no es gastado a trav\u00e9s de deporte.<\/p>\n

Es de importancia mantener controles peri\u00f3dicos con los profesionales del equipo, para evitar problemas emocionales y alimentarios, que nos lleven a reganar peso\u00a0 y no mantener el objetivo de baja de peso.<\/p>\n
